Please call the office to schedule an appointment. To best serve you, be prepared to provide your insurance information during your call. After our staff has registered you as a patient and scheduled your appointment, you will receive an email to register for your patient portal. Additionally, three days before your appointment, you will receive a text message from Phreesia, our check-in system. The check-in process will need to be completed before your appointment. For telehealth appointments, you will need to download the RingCentral app on your computer or smartphone in order to access your appointment. You will receive the meeting link in your patient portal the week before your appointment.
If you have a copy of your records and/or your ovulation tracking / cycle chart , please send those to us via email or the patient portal. This will help your consultation to be more effective and fruitful.
